Approved Is Not Paid | Tracing cross-bank payments from first principles reveals a system built on layered promises, not instant truth
Starting from the simplest possible model, two parties and one ledger, this post derives the full architecture of cross-bank payments through a series of questions. It maps the 7-layer execution pipeline, identifies the cross-cutting concerns that prevent it from breaking, shows who controls each layer and why the settlement layer is intentionally closed to private actors, and traces a single card payment end to end. The central insight is that authorisation and settlement are different events, that regulation is load-bearing rather than incidental, and that the system runs on delayed consistency held together by institutional trust.

<p>Take two parties, Alice and Bob. They want to transfer value. What actually has to happen?</p>
<p>If they share the same ledger, the answer is nearly trivial: find the relevant entries, decrement one, increment the other. But that simple case conceals everything interesting. The problem only starts when Alice and Bob are at different banks. That gap across ledgers is where payments become an engineering and trust problem.</p>
<p>Before mapping the landscape, it helps to be precise about what a bank account actually is. <q>You have £500 at Bank A</q> means: Bank A maintains a ledger, and that ledger contains an entry asserting you hold a claim worth £500. You don’t physically hold money; you hold a promise from Bank A. This isn’t pedantry; it becomes load-bearing the moment we ask why Bank B should trust anything Bank A does.</p>
<p>A payment, stripped to its minimum, satisfies three conditions: your balance decreases, someone else’s balance increases, and both parties agree this happened. Everything else, cards, apps, SWIFT messages, and cheque books, is mechanism. The conditions stay constant.</p>

<p>Not all mechanisms are equal. They differ across four axes:</p>

<h2 data-number="1.1" id="physical-bearer-transfer"><span class="header-section-number">1.1</span> Physical bearer transfer</h2>
<p><strong>Cash</strong>: you withdraw from your ledger claim, receive physical tokens, and hand them over. After withdrawal, no bank is involved. Immediate finality. Anonymous. This is the bearer-token model: possession equals ownership.</p>
<h2 data-number="1.2" id="same-ledger-transfers"><span class="header-section-number">1.2</span> Same-ledger transfers</h2>
<p><strong>Internal bank transfer</strong>: both parties bank at the same institution. One ledger, one update. Instant, cheap, no inter-bank coordination. The degenerate case.</p>
<h2 data-number="1.3" id="cross-ledger-transfers"><span class="header-section-number">1.3</span> Cross-ledger transfers</h2>
<p>Here the machinery becomes non-trivial. Bank A must reduce your balance <em>and</em> Bank B must increase the recipient’s, and the two banks must settle with each other.</p>
<p><strong>Bank transfer (sort code / account number)</strong>: you instruct Bank A to pay Bank B. Bank A sends a message via a clearing scheme. Bank B credits the recipient. The two banks reconcile through settlement, typically via accounts at the Bank of England. The UK runs three schemes differing mainly in speed and batch vs real-time:</p>

<p><strong>Direct Debit</strong>: the pull variant. The recipient holds a mandate and initiates collection. Runs on BACS. Reversible within a protection window, backed by the consumer guarantee that makes mandates viable at scale.</p>
<p><strong>Standing order</strong>: a scheduled push. Your bank sends a fixed amount on a defined cadence. Just a repeated bank transfer, with no new mechanism.</p>
<p><strong>Cheque</strong>: a written instruction, <q>Bank A, pay £X to Bob.</q> Bank B presents it for clearing. Delayed settlement, manual in origin, and declining in practice.</p>
<h2 data-number="1.4" id="card-payments"><span class="header-section-number">1.4</span> Card payments</h2>
<p>Cards are not a payment method in the same sense as the above. They are an authorisation and messaging layer built on top of bank transfers, with a two-phase structure:</p>

<li><strong>Authorisation</strong>: the terminal sends a message via the card scheme (Visa, Mastercard) asking your bank to reserve funds. No money moves.</li>
<li><strong>Settlement</strong>: hours or days later, the card scheme nets the day’s transactions and triggers actual inter-bank transfers.</li>
</ol>
<p>The card handles the UX. The money still moves through the banking rails.</p>
<p><strong>Mobile wallets (Apple Pay, Google Pay)</strong> add a tokenisation layer above cards: the PAN is replaced with a device-specific token. The wallet doesn’t move money; it delegates to the card it wraps, which delegates to the underlying bank transfer.</p>
<h2 data-number="1.5" id="pre-funded-and-external-value"><span class="header-section-number">1.5</span> Pre-funded and external value</h2>
<p><strong>Prepaid / e-money</strong>: you hold a claim on a non-bank ledger (an e-money institution rather than a licensed bank). Funds were moved there earlier, and spending draws down that stored value.</p>
<h2 data-number="1.6" id="cross-border-transfers"><span class="header-section-number">1.6</span> Cross-border transfers</h2>
<p>SWIFT is frequently described as the international payments network. This framing obscures the architecture: SWIFT is a <em>messaging</em> network. It carries instructions between correspondent banks. It does not move money. Settlement happens through nostro/vostro accounts, and the instruction and settlement remain separate events.</p>

<p>What this map doesn’t yet show is the coordination problem buried inside every cross-bank transfer: if Bank A debits you, why should Bank B trust it will be compensated? That single question forces the existence of clearing systems, settlement systems, and central banks.</p>
<h1 data-number="2" id="the-7-layer-model"><span class="header-section-number">2</span> The 7-Layer Model</h1>
<p>The standard framing, <q>money moves from A to B,</q> collapses multiple distinct systems into one opaque event. Working from first principles, there are seven separable layers, each with a distinct function:</p>

<p>These aren’t stages every payment visits in sequence. They’re the full vocabulary. A cash withdrawal exits at layer 7 on one side and never touches layers 4–6. <p>A few things this structure makes explicit that the simpler framing conceals:</p>
<p><strong>Interfaces are interchangeable at the back end.</strong> A bank app, a card tap, and a wallet are different UX surfaces feeding the same downstream rails. The interface layer shapes user experience; it doesn’t determine which settlement infrastructure the payment uses.</p>
<p><strong>Messaging is not settlement.</strong> SWIFT transmits instructions. It does not move value. A SWIFT message can be sent and acknowledged while the corresponding settlement is delayed, queued, or, in failure scenarios, never completed. The two are causally linked but temporally separable.</p>
<p><strong>Clearing is compressible.</strong> Real-time gross settlement systems like CHAPS bypass net clearing entirely, and each transaction settles individually. Batch systems like BACS accumulate obligations and net them before settlement. The clearing layer exists on a spectrum from fully present to nearly invisible.</p>
<p><strong>Settlement is the ground truth.</strong> It is the only layer where value genuinely moves between institutions, in central bank money that carries no counterparty risk. A balance change in Bank A’s ledger is a commitment made in anticipation of settlement, not a record of it. Bob is paid when the central bank reserves move, not when the app shows a green tick.</p>
<h1 data-number="3" id="cross-cutting-concerns"><span class="header-section-number">3</span> Cross-Cutting Concerns</h1>
<p>The 7-layer model describes the pipe. It says nothing about why the pipe doesn’t break.</p>
<p>There are two classes of system that don’t fit neatly into any single layer. They don’t execute between layers 3 and 4, or 5 and 6. They observe and influence multiple layers simultaneously.</p>
<h2 data-number="3.1" id="provider-designed-overlays"><span class="header-section-number">3.1</span> Provider-Designed Overlays</h2>
<p><strong>Fraud prevention</strong> doesn’t sit between authorisation and messaging. It interrogates the interface layer (is this device recognised?), the authorisation layer (does this pattern warrant a block?), the messaging layer (does this instruction look anomalous?), and the ledger layer (can this be reversed post-settlement?). A fraud system that only queried one layer would catch a fraction of what it needs to.</p>
<p><strong>Liquidity management</strong> shapes outcomes across three layers. It can cause a rejection at authorisation. It affects clearing because net obligations depend on transaction volumes and timing. It constrains settlement because central bank reserves must be present when settlement windows open. A payment can be fully authorised and still fail to settle if the sending institution hits a reserve shortfall.</p>
<p><strong>Card guarantee schemes</strong> are a structural overlay on clearing and settlement. Visa and Mastercard guarantee merchant payment even if the issuing bank fails to settle. This separates merchant risk from the underlying settlement infrastructure, an explicit overlay on layers 5 and 6, funded through interchange fees.</p>

<p>The temptation is to treat regulatory requirements as pipeline steps, with a compliance box sitting somewhere between layers 3 and 4. That framing breaks as soon as you trace a specific obligation. KYC operates at the interface layer (onboarding), the authorisation layer (is this person permitted to transact?), and the messaging layer (does this instruction trigger a screening match?). Settlement finality is a legal property of the settlement layer that affects how every upstream layer is designed. SCA reshapes the interface layer entirely.</p>
<p>The 7 layers describe vertical execution, how a payment moves. Regulatory requirements describe the gravitational field within which that movement is permitted to occur.</p>

<p>Many rules are principles-based. <q>Have adequate fraud controls</q> is not a specification; it is a regulatory expectation you must justify after the fact.</p>

<p>The BIN is the first lookup. Those six digits on your card tell Visa which bank to route to. The system works because every message carries structured identity metadata. Without it, cross-institutional coordination is impossible.</p>
<h2 data-number="6.2" id="before-your-bank-sees-anything"><span class="header-section-number">6.2</span> Before your bank sees anything</h2>
<p>Most accounts of card payments skip this phase entirely. Before your bank receives the authorisation request, roughly 100–500ms of processing happens across several actors:</p>
<p><strong>Capture.</strong> Card number or device token. Apple Pay transmits a DPAN, so your real PAN never leaves your device.</p>
<p><strong>Luhn check.</strong> Client-side, catches typos. Non-authoritative, and mainly there to avoid bad requests.</p>
<p><strong>Gateway fraud screen.</strong> IP reputation, velocity, device fingerprint. The gateway can block the transaction here, before the bank is involved at all.</p>
<p><strong>Routing decision.</strong> Which acquirer? Which network? Can be dynamically optimised.</p>
<p><strong>3DS challenge.</strong> If SCA is triggered (PSD2-mandated), the issuer authenticates you via OTP or biometrics.</p>
<p><strong>Auth message construction.</strong> The gateway builds an ISO-format payload containing amount, merchant category code (MCC), token/card, 3DS result, and risk indicators.</p>
<p><strong>Transmission.</strong> Gateway → Acquirer → Visa → Issuer via BIN lookup.</p>
<p>Authorisation is the <em>last</em> step in this pipeline, not the first. By the time your bank sees the request, it has passed through multiple layers of filtering and enrichment. Multiple actors can stop the payment before the bank ever sees it.</p>
<h2 data-number="6.3" id="authorisation"><span class="header-section-number">6.3</span> Authorisation</h2>
<p>The issuer checks: balance sufficient? Fraud score acceptable? Card active? SCA satisfied? If yes, it places a hold on £1,000.</p>
<p>The hold is not money movement. It is a promise to settle later.</p>
<h2 data-number="6.4" id="confirmation"><span class="header-section-number">6.4</span> Confirmation</h2>
<p>Approval propagates in reverse: Issuer → Visa → Acquirer → Gateway → Merchant. The merchant releases goods based on Visa’s guarantee. What Visa is selling at this moment is trust, because settlement has not happened yet.</p>
<h2 data-number="6.5" id="clearing"><span class="header-section-number">6.5</span> Clearing</h2>
<p>End of day. Visa nets across millions of transactions and calculates aggregate positions: <q>Issuer A owes Acquirer B £X net.</q> This collapses the settlement load significantly, from one transfer per transaction to one net transfer per bank-pair per day.</p>
<h2 data-number="6.6" id="settlement"><span class="header-section-number">6.6</span> Settlement</h2>
<p>This is the only moment value actually moves between banks.</p>
<p>Your issuer’s reserve account at the Bank of England debits. The acquirer’s reserve account credits. This runs on RTGS. It is final and irreversible.</p>
<h2 data-number="6.7" id="ledger-update"><span class="header-section-number">6.7</span> Ledger update</h2>
<p>Both banks close out their internal records:</p>

<li>Acquirer: merchant +£1,000</li>

<p><q>Approved</q> means the decision was made. <q>Paid</q> means settlement ran. These are different events, separated in time and in institutional responsibility.</p>
<p>A card payment is split in time and responsibility. The decision is instant. The reality is delayed.</p>
<h1 data-number="7" id="card-networks-vs-bank-payment-schemes"><span class="header-section-number">7</span> Card Networks vs Bank Payment Schemes</h1>
<p>When you use your bank app to send money, you’re still using a network, just a different type.</p>
<p><strong>Card networks (Visa, Mastercard)</strong> are designed for merchants accepting payments. They identify the payer indirectly: card number → BIN → issuing bank. The payer and merchant may never have a prior relationship, so the network exists to bridge unknown counterparties, standardise messages, and guarantee payment to merchants. Authorisation happens now; settlement happens later.</p>
<p><strong>UK interbank payment schemes (Faster Payments, CHAPS, BACS)</strong> are designed for account-to-account transfers where both parties are explicitly known upfront via sort code and account number. No dynamic discovery needed, no card scheme abstraction. Faster Payments settles near-instantly; CHAPS is same-day RTGS; BACS batches and settles over three working days.</p>

<p>Card networks exist because without them, N banks would require N×(N-1) bilateral connections to interoperate. With a network, each bank needs one connection to the scheme. Networks also provide a shared message format (ISO 8583 for cards, ISO 20022 for modern bank payments), shared trust frameworks, and liability rules. They aren’t just routers; they are rule systems that turn a mesh of bilateral relationships into a single shared system.</p>
<h1 data-number="8" id="what-remains-open"><span class="header-section-number">8</span> What Remains Open</h1>
<p>The payments landscape reads as a stack of closed systems. But the picture is more nuanced.</p>
<p>The settlement layer is closed to private actors by design. Central bank access requires regulated status, and this is a feature, not a limitation. If anyone could settle, the system would lose finality and trust.</p>
<p>The layers above settlement are progressively more open. Stripe, Adyen, and Revolut have built substantial businesses operating at the interface and orchestration layers, partnering downstream for the parts they cannot own. Open Banking adds a further dimension: licensed third-party providers (TPPs) can initiate payments via regulated APIs, using Faster Payments under the hood, without needing to become a bank.</p>
<p>The pattern is consistent across major payment innovations. The innovators live at the edges. The central bank remains unreachable to private actors, by deliberate design. What looks like regulatory overhead from the outside is the load-bearing structure of a system that processes hundreds of billions of pounds annually.</p>
<p>The most interesting architectural feature is the consistency model. This is a globally distributed system running on delayed consistency. Millions of transactions are approved in real time, while settlement confirms hours later in batches. Participants operate on promises while waiting for truth to land. The system functions because institutions trust intermediaries, especially Visa’s guarantee and RTGS finality, more than they need to verify each transaction individually.</p>
<p>What most people call <q>payment,</q> the tap or the confirmation screen, is the opening event of a process that has not finished yet. The closing event is a debit at the Bank of England.</p>
